To enroll in Summer Camp 2018, please complete this Registration Form along with the Enrollment Agreement Form. Payment for each camp session is due at the time of Registration. You will be able to choose the weeks desired and be able to pay in full after completion of this form. Your camper will no be permitted to attend camp until all Forms are completed in their entirety and payment is made.

There will be a one time $25 registration fee add to the final cost.
If attending, select the weekend your camper will be needed to attend.
Aftercare - Aftercare is available daily from 4:00pm-6:00pm. I have placed an “√” (checkmark) in the box above to designate which camps my child will be attending aftercare. The cost of aftercare is $28.00 per week which is in in addition to the weekly camp cost. By doing this, I acknowledge that I am registering my child to attend aftercare for the weeks indicated and agree to assume responsibility for payment of tuition for each weekly camp session designated irrespective of whether my child misses one or more days of any weekly camp or aftercare session.
FOR CAMPS: The Responsible Party is responsible for paying all monies listed above whether the student attends the programs/classes/sessions listed above and for which payment is due. On or before 30 calendar days before the child’s first camp will start a parent can send written notification to cancel the camp enrollment and receive a refund less ten percent (10 %) of the tuition fee back (no refund of registration fee).
1. If any payments due under this agreement are not paid when due, the student may be denied the right to attend classes.
2.If the Responsible Party wishes to have the Student picked up by someone other than those specifically listed in this agreement, the Responsible Party will provide Foreign Language Immersion School with written authorization allowing the third party to pick up the child.
On behalf of myself and my Student (child/children) and acting as the Responsible Party I understand that it is my sole responsibility to have the appropriate person physically present at and in the facility to pick up my Student (child/children) and I agree to indemnify and hold harmless Foreign Language Immersion School, its owners, operators, instructors, employees, agents and servants from any and all responsibility and liability in regards to my Student (child/children) being picked up from and/or leaving the facility.
3. If a Student, Parent, Responsible Party or anyone associated with the Student, in the sole judgment of the teacher, teaching assistant or administrative staff, causes disruption in the classroom or facility or in any other way detracts from the other student’s ability to maximize their learning opportunities, the Student may be dismissed from the program. This agreement may be terminated by Foreign Language Immersion School for any reason and if terminated by Foreign Language Immersion School the Responsible Party will be refunded for any classes/sessions not attended, however will not be refunded for any enrollment fees and/or classes/sessions previously taken. No prorated refunds will be provided for any partial weeks. Attended.
4. The Responsible Party acknowledges and agrees that the Student may be video taped, filmed or photographed during classroom activities, and hereby authorizes Foreign Language Immersion School to use the parent’s and student’s name and any resultant likeness of the Student in its advertising or promotional materials, e-mail, Facebook or otherwise or for any future promotions or publications online, in print or otherwise or otherwise without compensation and gives permission for their Student to take part in all activities associated with Foreign Language Immersion School.
5. I/we (responsible party) understand the nature of the physical, athletic and all other activities that our student/child will participate in, and attest that the student/child is qualified, in good health and in the proper physical and mental condition to participate in such activities.
By completing this registration and making payment through the FLIS checkout, I affirm that, except as indicated in this Application Form, my child is in good health and able to participate in all activities. With the understanding that safety standards will be met, I release the Foreign Language Immersion School from possible claims for injury to person or property, which may arise from participation in activities and hereby covenant and agree to hold harmless the Foreign Language Immersion School, its employees, agents or representatives from any claim, liability or expense arising out of, or in any way connected to, any alleged incident or injury resulting from such participation. In the event of medical treatment, if neither parent nor emergency contact is available, I give the Foreign Language Immersion School permission to transport my child to a medical emergency room for treatment. I also give permission for my child to participate in any onsite summer camp activities offered. I also hereby acknowledge receipt of the policies and procedures for summer camp and acknowledge that I understand them and agree to abide by same.
